We added the Bill Code filter to your report and can confirm that some Bill Codes are not a filter option in the drop-down (see attached screenshot titled "Filter" for reference). Due to the Bill Code Rates Report being built using Service Explore, if a Service does not have a Bill Code attached, then the Bill Codes will not be an option in the filter drop-down and will not pull into your report.
We recommend building your report using Service Code Explore instead to capture all the Bill Codes found in Accounting Settings > Bill Codes as there will always be Bill Code and Pay Code configurations on the Service Code to determine how the item is paid or billed.
We've attached a screenshot titled "DE 2.0" as an example of the Bill Code Rates Report built using Service Code Explore that captures all the Enabled Bill Codes found in your Accounting Settings. You can then add the Bill Rate and Bill Rate Start Date fields and filter for the effective start date that you desire (see attached screenshot titled "Results" for reference).

I created a report to pull bill codes and their rate. In the report I wanted to filter bill code rates by effective start date. The results was not giving all the bill codes rate effective mon dd, yyyy. would you have a similar report that will give me the results I need?
Basically I wanted to pull a report of bill code/ bill rates with their effective start date.
