How do I view HCCSS Referral Information in a Client Profile/Service?

When a Referral is processed, its information can be retrieved from within the Service and may update the Client Profile as well depending on the scenario and your System Settings for Intake.

Referral Information on the Client Profile

You may have created a new Client Profile from the Referral, or updated an existing Client Profile with a new Referral. If so, the following tabs contain fields updated by the Referral.

  1. Client Profile>Demographics. The following fields may be auto-populated by the Referral.

    • General tab
      • External ID
      • First Name/Last Name
      • Phone
      • Birthday
      • Gender
      • Language
    • Address tab
      • Address
      • Address Line 2
      • City
      • State
      • Zip Code
      • Country
    • Other Fields tab
      • Health Card
      • Health Card Version



  2. Client Profile > Care Documentation > Medical History.



  3. Client Profile>Overview>Risk Assessment.



  4. Client Profile>Coordination>Contacts.

Referral Information on the Service Profile

If you have created a new Service from the Referral, or updated an existing Service with a new Referral, the Service Profile will be affected in the following places.
 
  1. Client Profile>Services>Service Overview
    • Service Name
    • Start Date
    • Projected End Date
      Emergency Response Level
    • Address
    • Address Line 2
    • City
    • State
    • Zip Code
    • Country



  2. Scroll down the Service Overview page to view/edit the latest Referral Info.



  3. Scroll down still further and click on Referrals to see the history of Referrals for the Service



  4. Click View for any Referral to see the full information received.
