1. Demographic Information
General demographic information will be populated (name, phone #s, address, HCN etc.) into the client profile. As the user reviews this tab, they can also use the opportunity to assign this new client a "Group" for filtering purposes throughout the software.
Attachments are not pulled through from HPG. Users can use our drag and drop functionality to store any of those attachments in the client profile.
Users can populate the risk section of the client profile with any relevant risk codes that are outlined on the referral.
When a Referral/Update is processed, the Emergency Response Level (ERL) will be pulled into the Risk Assessment tab in AlayaCare.
Contacts are automatically populated from the referral. Users can take this opportunity to rank these contacts within the emergency contacts tab.
6. Service Profile
The following fields are auto-populated from the referral:
- Service Name
- Start Date
- Projected End Date
Users can complete the service profile by including:
- Funder (payor)
- Service Code
- Primary Employee (where applicable)
- Activity Codes (where applicable)
- Reports (where applicable)
- Skills Needed (where applicable)
7. Referral Information
At any point, users can review the latest referral information and/or the full referral PDF from within the service profile.
