Forms are not showing for caregivers.

Forms may not be visible to caregivers if they have not been properly attached to a specific visit or service. Ensure that the form is correctly set up, linked to the relevant visit or service, and that the appropriate roles have access to submit and view the forms.

Solution:

1. Create the form

You can access the Form Builder by clicking Build on a Form in Form Settings. For detailed guidance, refer to the How do I use the Form Builder?

2. Add the form to a visit or service

Attach the form to the specific visit or service where it needs to be available to caregivers. You can also add a form as a Service Task to be completed before clocking out. Follow the instructions in the How do I add a form to a visit or service? to ensure the form is linked properly.

3. Test in staging environment

Test the form upload and accessibility in your staging environment before applying changes to the live environment. This helps ensure that everything is set up correctly and prevents issues in the production environment.

4. Verify form availability

Check that the form is correctly linked to the visit or service and is visible to caregivers.
