How to add an assessment within AlayaCare Assessments

Learn all about adding an assessment within AlayaCare's Assessments module.

Add New Client in AlayaCare

Log into AlayaCare and head to Client Menu > Client List. The "Add Client" button should be visible to the user.

If the user does not have permission to create a client, the user’s roles should be reviewed. In addition to being assigned to an AlayaAssessment role, the user must also be assigned to a role that provides access to complete other activities within AlayaCare Cloud. To start, click on "Add Client" and use the search feature to validate that the client does not already exist in the system. If no results are returned based on the search information, click on "Add Client" and the system will then create a new client record. Enter client information as required by your agency. The following fields will transfer from the client’s core health record to the AlayaCare Assessments module:

  • Client first and last name
  • Date of birth
  • Client unique identifier (health card number)
  • Case record number
  • Sex at birth
  • Marital Status
  • Postal Code

For more information about the integration of this data or configuration required to set up this data, refer to AlayaCare Assessments Integrated Data Elements. Once all information has been entered into the client’s health record, save this information.

Launch AlayaCare Assessments

In order to launch the client “in context”, navigate to the client record in AlayaCare Cloud. Once in the client record, go to the Care Management section from the top navigation bar, then click on the "Assessments" button from the left hand menu listing.

Based on system permissions, the user may have access to two menu options called Assessments. In order to access AlayaCare Assessments, select the option that has a launch arrow at the start of the label.

Once you are in, click on “Launch Assessments”, and a new tab will open with the AlayaCare Assessments module. The first time that the module is launched for the user, it will display the Assessment Page. The data displayed in the left hand pane may vary based on the interRAI instruments in use.

To start a new interRAI assessment, select "Start New Assessment" in the upper left corner. This will open a command box where the user will select the assessment type that is to be completed. The list of assessment types will vary based on your agency and your permissions.

Select the correct option and click on "Create". The system will create the assessment based on this selection.

Navigate to the tile to open the assessment.
