Form Context Overview
Through Form Context Fields, system administrators are able to add, re-label and remove demographic fields for: employee profiles, client profiles, staffing position/facility profiles, employee contacts, client contacts, service profiles and funder profiles.
Navigation: Settings - > All Settings - > Form Context
Important Note: Please ensure that you do not remove any critical demographic fields such as name or address fields as that may affect different integrations in the system.
1. Remove Form Context Fields
Step 1: Navigate to the field you would like to remove
Step 2: Un-check this field under the applicable demographic group
In the example below, the fields "Health Card Number" and "Health Card Version" are turned ON for Client Demographics and turned OFF for Employee Demographics:
2. Re-Label Existing Form Context Fields
Step 1: Navigate to the field you would like to re-label
Step 2: Select "Options" under the field and input the new field name under "Label"
3. Add New Form Context Fields
Step 1: Click "Add Attribute"
Step 2: Input the following details:
- Tag - this field will auto-populate and should not be adjusted
- Input Type
Step 3: Check the fields ON under the applicable demographic group
Form Context on Client Profiles
Client profiles are created with a default profile based on Form Context fields. The default fields for a client profile are listed below:
- First name (required)
- Last name (required)
- Address Suite
- Preferred email
- Phone- cell
- Phone- main
- Phone- other
- Phone- personal
- Health card
- Health card version
