How do I add Display Logic to a Form?

Display Logic allows you to configure certain Form fields to appear only when a caregiver enters specific responses to other questions on the Form. For instance, if you have added a Checkbox to the Form asking the caregiver to check the box if the Client smokes and you then need to ask how many cigarettes the Client smokes a day, you can set Display Logic so that the second question only appears when the box indicating that the Client smokes is checked. 

You can add Display Logic for a question on a Form once you have added at least one component via the Form Builder. The +Add Display Logic button cannot be clicked when adding the first component to a Form.

Add Display Logic for a Component 

After dragging and dropping the component into the Form click +Add Display Logic in the Edit Component box.

Use the fields in the resulting dialogue box to define the logic that will control whether or not the question will be displayed when a user is completing the Form

Begin by reading the informational message: "Display [component Label] only if." You will use the dropdowns below to select the conditions in which this field should appear.

Use the first dropdown to choose whether the component should be displayed if Any of or All of the items below are true. 

Use the next dropdown to choose one of the components that have already been added to the Form and select what the response be in order for the component you are creating to be displayed. You can select the question from the dropdown and then select is checked from the adjacent dropdown. That way, the second question will display only if the user checks the box.

warning_icon.jpg Note: while you can add Display Logic to an Advanced Component based on Basic Components, you cannot select an Advanced Component to use for determining the Display Logic for another component. You will only be able to select Basic Components from the dropdown.

If a Textbox or Comment Box question is selected, you can choose contain or does not contain and enter the keywords you wish to filter by in the text field.

If a Checkbox question is selected, choose checked or is not checked

If a Dropdown question is selected, choose equals or does not equal and select one of the options configured for the component.

If a Multi-Select question is selected, choose equals or does not equal and select one or more of the options configured for the component.

If a Number question is selected, choose equals, does not equal, is less than, or is greater than and then enter the number you wish to base the logic on in the third field.

If a Time Picker is selected, choose equals, does not equal, is less than, or is greater than and enter the time you wish to base the logic on in the third field.

If a Date Picker is selected, choose equals, does not equal, is less than, or is greater than and enter the date you wish to base the logic on in the third field.

If a Picture, Signature, or Drawing question is selected, you can choose is set or is not set (based on whether a response has been entered for the question).

To add more conditions, click +Add Field

Once you have finished adding Display Logic for the question, click Save

The Display Logic will appear in the Edit Component box.

Preview and Edit Display Logic for a Form

To preview a Form's Display Logic, click Preview from the Form Builder

Complete the Form in Preview to check that the Display Logic is working as expected. 

If you need to adjust the logic for a question, click Build to return to the Builder and click the edit_pencil_icon.png icon on the component whose logic you wish to edit.

Next, click edit_pencil_icon.png to change the Display Logic.

Once you have finished editing the logic rules, click Save to save your changes.

To delete Display Logic for a question, click trash_icon.png next to Display Logic.
