Forms can be completed and submitted for Clients from the mobile app as well as the web app. You can add a new Form from the Form History tab, fill out a Sticky Form in the Client Dashboard, complete a Form from your Service Tasks list, or fill out a Form attached to a Task.
Add a New Form for a Client
To add a Form for a Client on mobile, click on a Visit in your Schedule or a Client from your Caseload and navigate to the Form History tab.

From here you will see a list of all Forms that have been previously submitted for the Client. Click on the Form Name to view a PDF of the submitted Form.

To add a new Form, click the + button.

Select which Form you wish to add for the Client. Use the search bar to search for the Form by name.

Complete the Form fields for the Client. Click Next to move on to a new section.

When you have finished filling out the Form, click Submit/Save.

Create/Update a Sticky Form for a Client
If the Form you need to complete for a Client is a Sticky Form (if Display Report in Client Dashboard was checked when the administrator created the Form), the Form name will be displayed as a separate tab on the Client's profile on the mobile app.
To fill out a Sticky Form, swipe to the tab in the Client Dashboard with the Form name and tap Create to complete the Form fields.

If the Form has already been completed for a Client, you will also be able to see the latest completed version of the Form. To update the information in the Form fields, tap Update and then complete the Form.

When you have finished, click Submit/Save.

Complete a Form as a Service Task
Clock in to a Visit and swipe to the Service Tasks tab.

Any Forms that need to be completed as part of the Visit or Service will be listed here. Tap the box next to the Form you wish to complete.

The Form will automatically open. Complete the relevant fields and click Submit/Save.
After submitting the Form, you will be returned to the Service Tasks list. The Form will be marked as complete and the date and time of submission will appear beneath it. The counter for the Forms section will also be updated.
Complete an Assigned Form in your Tasks
Navigate to the Tasks tab to view all Tasks assigned to you for all Clients.

Tap to open the Task with the Form attached.

Tap Fill Form.

Complete all necessary fields of the Form. Any Form fields meant to be completed by the person who created the Task will be read only. When you have finished, click Submit/Save.
