If you wish to assign a Form to an Employee as a Task, you must first create a Task-type Form and then create a Task with the Form attached.
Create the Form
To begin, click +Add Form from Form Settings.
In the Add Form dialogue box, select Task as the Type. If you wish to attach the Form to an Event Task, select Event Task as the Type. Learn more about Event Tasks.
Complete the other fields in the dialogue box and then click Save.
In the Form Builder, drag and drop the components you wish to use into the Form. For each Basic Component you add to the Form, you will have the option to mark it as a Task Field in the Edit Component box.
If Task Field is checked for the component, this field will appear in the Add Task dialogue. It can be completed upon Task creation and will appear as read only on the Form.
When you have finished building your Form, click Publish to change its status to Active.
After creating and publishing the Form, you must create a Task with the Form attached. From the Tasks tab on the dashboard or Tasks on the Client or Employee profile, click +Add Task.
Click Yes when asked whether you wish to attach a Form.
In the Form field, start typing to select the Form you wish to attach to the Task. You can select any Active Form of Type Task.
The fields for which you checked Task Field when building the Form will appear at the bottom of the dialogue box. If these fields are required, you must complete them in order to click Save.
Once you have added the Task, it will appear in the list of Tasks on the Client's profile and on the assigned Employee's profile.

You can fill out the Form directly from the by clicking the arrow next to View and selecting Fill Form.
Any Task Fields on the Form will be read only.
When the Employee submits the Form, a tooltip will appear on the Task with the name of the Employee who submitted it and the time of submission.

- In the View dropdown of a Form Attached Task, click Edit.
- Once the Form attached to the Task has been submitted, a tooltip will appear next to Task Name with the name of the Employee who submitted the Form along with the date and time of submission.
