Once a Care Plan has been created and published for a Client, caregivers can review a Client's current Active Care Plan from the mobile app. They can also complete/resolve and archive Care Plan Diagnoses, Goals, and Interventions. ACLs are assigned to roles to allow them to view the Care Plan and to complete/resolve items within the Care Plan.
To review a Client's Care Plan, open a Visit in your Schedule or a Client in your Caseload and swipe to the Care Plan tab.

Diagnoses on Mobile
The first Care Plan items displayed on the Care Plan will be the Client's Diagnoses.

For each Diagnosis, you will see the item Name, Start Date, End Date if applicable, and the date when the item was last updated (along with the name of the user who updated it). The icon will appear if any Tags are attached to the Diagnosis. The Client's Primary Diagnosis will be marked with a . Diagnoses are characterized by Type (for example, Nursing).
Click the three dots (iOS: , Android: ) on the right to view the actions you can perform on the Diagnosis.

You can resolve or archive a Diagnosis directly from the Care Plan on mobile. Select Filter if you wish to filter the Care Plan to only this Diagnosis and any linked Goals or Interventions. If any Tags are attached to the Diagnosis, you will also have the option to View Tags.
Goals on Mobile
The Client's Goals will appear after the Diagnoses on the Care Plan.

For each Goal, you will see the item Name, Start Date, End Date, and the date that item was last updated (along with the name of the user who updated it). The icon will appear if any Tags are attached to the Goals. The Client's Primary Goal will be marked with a . Goals are categorized by Type (for example, Physiotherapy).
You will also see the names of any Diagnoses that the Goal is linked to.

If Percentage is the Progress Tracking Method for the Goal, the Percentage of completion for the Goal will be displayed on the Care Plan.

If Per Visit with Comments is the Progress Checking Method, the time the Goal was Last Checked during a Visit will be displayed on the Care Plan.

Click the three dots (iOS: , Android: ) on the right to view the actions you can perform on the Goal.

You can complete or archive a Goal directly from the Care Plan on mobile. Select Filter if you wish to filter the Care Plan to only this Goal and any linked Diagnoses or Interventions.
If Per Visit with Comments is selected as the Progress Tracking Method for the Goal, you can also select View Last Check.

If any Tags are attached to the Goal, you will also have the option to View Tags.
Interventions on Mobile
Interventions are the last items to appear on a Client's Care Plan.
iOS: Android:
Interventions are categorized by Type (for example, Dietary/Nutrition, Household Management, Nursing, etc.) For each Intervention, you will see the item Name, Start Date, End Date (if known), details about how often the Intervention should occur (the Intervention Frequency), and the time when the item was updated (along with the name of the user who updated it). The icon will appear if any Tags are attached to the Interventions. If you must complete the Intervention (or leave a comment) before clocking out of a Visit, it will be marked with a *.
If an Intervention is linked to another module in AlayaCare (for example, Forms or Medications), these details will also be included on the Intervention.

If you are required to complete a Form as part of the Intervention, a hyperlink to the Form will appear on the Intervention.

You will also see the names of any Goals on the Care Plan that are linked to the Interventions.
For caregivers in the U.S., the 485 icon indicates that the Intervention will be included on the 485.
If you are required to capture the time spent on an Intervention when completing it, the clock icon will appear on the Intervention.

You can complete or archive an Intervention directly from the Care Plan on mobile. Select Filter if you wish to filter the Care Plan to a single Intervention and any linked Diagnoses or Goals.

If any Tags are attached to an Intervention, you will also have the option to View Tags.
Filtering Options on the Care Plan
You can filter the information on a Client's Care Plan in multiple ways (by Start Date, Care Plan item, etc.). To explore different filtering options, click the filter button in the bottom right corner of the screen.

Choose which filters you wish to apply to the Care Plan.
iOS: Android:
Tap the Status field to select which item Statuses (Active, Completed/Resolved, and/or Archived) you wish to filter the Care Plan to.
Tap the Care Plan Item field to choose which Care Plan item to filter the Care Plan by. This will filter the Care Plan to show only the selected Diagnosis, Goal, or Intervention and any items that are linked to it.
Use the Date from and Date to fields to select the Start and End Dates for the items you wish to include in the filter.
If you wish to filter by Department, Service, or Tag, tap the Department, Service, or Service fields to select.
