For a form to appear in the service tasks of a visit, it must be added to either the service for the visit or the visit itself.
Adding a form to a service
To add a form to a service, navigate to a client profile (or facility profile) and click on the services tab.
Click edit service on the service to which you wish to add a form. To add a new service with a form attached, click create a service.
In the resulting dialogue box, click under forms to select the form you wish to attach to the service. All active forms will be available for selection.
Note that you can add multiple forms to a single service. Click to add more forms. When you have finished, click save.
When scheduling visits for this service, the name of the form will appear under forms in the new client visit dialogue. Click the x if you wish to remove it.
Adding a form to a visit
To add a form to a visit, navigate to a scheduling page in AlayaCare and create a new visit by selecting a time frame on the calendar (or clicking the +add visit(s) button).
To add edit a visit that has already been created, click on the visit in the calendar and then select the edit tab in the resulting dialogue.
Under the forms field, you will see any forms that have been added to the service. Click x if you need to remove a form.
To add a a form to the visit, start typing the name of the form you wish to add to the visit in the forms field. After selecting the form, it will appear underneath the forms field.
Note that you can add multiple forms to a single visit. When you have finished adding to the visit, click update visit/create visit.
Viewing the service/visit forms in service tasks
Once the visit is created, any attached forms will appear in the visit's service tasks under forms.
After you are clocked in to a visit, you can complete and submit forms or add more forms as service tasks. Select view definition to preview the form as created in form settings or fill in to complete the form. You will also be able to see if a draft of the form has been saved.
