How do I add Care Plan Items (Diagnoses, Goals, and Interventions) to a Care Plan Library?

Care Plan Libraries follow a similar structure to an individual Client Care Plan and can be composed of Diagnoses, Goals, and Interventions.

When adding a Diagnosis, Goal, or Intervention to a Care Plan Library, the fields will mostly be the same as when adding an item to a Client Care Plan. However, you will not be able to enter Start and End Dates, select a Service(s), or have the option to choose Single Items when linking to the Medication, Infusion, Vitals, or Wound Care modules on an Intervention.  

Add a Diagnosis to a Care Plan Library

To add a Diagnosis, click the add_care_plan_items_symbol.png button.

In the dialogue box, enter the Diagnosis Name. Tag and Department are optional fields.

When you have finished, click Save. The Diagnosis will be added to the Care Plan Library. 

Add a Goal to a Care Plan Library

To add a Goal, click the add_care_plan_items_symbol.png button.

In the dialogue box, complete the following fields:

  • Name: enter the name of the Goal you wish to use in AlayaCare. This field is mandatory. 
  • Description: use this comment box to provide further information about the Goal if necessary. 
  • Type: select the Type from the list of Goal Types configured by your agency. Standard Goal Types include Nursing, Physiotherapy, Occupational Therapy, Speech Therapy, Personal Care, and Client.  This field is mandatory. 
  • Tag: use the multi-select to choose any Care Plan Item Tags you wish to link to this Goal. Learn more about Entity Tags. 
  • Related Diagnoses: select any Diagnoses from the Care Plan Library that you wish to link to this Goal. 
  • Department: if necessary, select a Department from the dropdown to add to the Goal. Only one Department can be selected from the dropdown list.
  • Progress Tracking Method: choose from three options: None, Percentage and Per Visit with Comments. With None, caregivers will not track the interim progress of the Goal from Visit to Visit, but will still have the option of marking the goal as complete in the Care Plan. With Percentage, caregivers can enter a percentage of completion to track progress toward the Goal. With Per Visit with Comments, caregivers will be prompted to mark the Goal as complete at the end of each Visit. 

When you have finished, click Save. The Goal will be added to the Care Plan Library. 

Add an Intervention to the Care Plan Library 

Click the add_care_plan_items_symbol.png button to add an Intervention to the Care Plan Library.

In the dialogue box, complete the following fields:

  • Name: enter the name you wish to use for the Intervention. This field is required. 
  • Description: use the comment box to provide a description of the Intervention if necessary. 
  • Type: select the Intervention Type. The dropdown will contain a list of Intervention Types configured by your agency.  Standard Intervention Types include Personal Care, Nursing, Therapy, Support, Dietary/Nutrition, Education, Mobility, Equipment/Supplies, Household Management, Medication, Would Care, and Infusion. 
  • Tag(s): use the multi-select button to choose any Care Plan Item Tags you wish to link to this Intervention. Learn more about Entity Tags.
  • Related Goal(s): select one or more Goals from this Care Plan Library that this Intervention is intended to help the Client achieve. 
  • Department: if necessary, select a Department from the dropdown to link to the Intervention.  Only one Department can be selected from the dropdown list.
  • Required for Visit Completion: check this box if caregivers must complete the Intervention or add a comment explaining why it was not completed before they can clock out of a Visit. 
  • Capture Time Spent on Intervention: check this box if you wish to have the time it takes to complete the Intervention recorded during Visits. 
  • Include in 485: check this box if the Intervention should be included on the 485 (US agencies only). 
  • External Link: use this field if you wish to include a hyperlink to an external website to help the caregiver with assisting the Intervention. Enter a Label for the link as well as the URL. 
  • Link to AlayaCare Module: use this field if you wish to link the Intervention with another module in AlayaCare, such as Forms, Vitals, Medication, or Infusion. For Forms, you must specify the Form the Intervention should be linked to. 
  • Intervention Frequency Type: select the Frequency Type to determine how often the Intervention should occur. The possible Frequency Types are Per Request, Per Visit, Daily, Weekly, and Monthly. If you select Daily, Weekly, or Monthly, you will need to enter additional details about how often the Intervention should be scheduled for the Client. When you have finished, click Save. The Intervention will be added to the Care Plan Library.

Make Changes to a Care Plan Item 

To make changes to a Diagnosis, Goal, or Intervention you have added to a Care Plan Library, hover over the item.

To edit the item, select the icon.

To delete an item in a Care Plan Library that has not been published, click the trash_icon.png icon.

Use the filter_icon_care_plan.pngicon to filter the Care Plan Library to the item and any linked items.
