When a Client has a wound that needs to be treated by a care provider, you can create a Wound Care Profile to describe the nature of the wound and specify how it should be treated.
To add a new Wound for a Client on web, go to Care Documentation>Wound Care on the Client's profile.
You will see all Wounds added for a Client listed by Wound Name, Wound Type, Wound Acuity, Wound Location, Frequency, Count of Visits, Total Visits Permitted, Treatment Status, Latest Healing %, Latest Treatment Date, Latest Swab Date, Wound Care Entered Date, and Wound Care Created By (name of the Employee). Click View or on the Wound Name to review the Wound details.
You can filter the list by Wound Type and Treatment Status (Active or Complete).
To add a new Wound, select +Add Wound Care.
In the Add Wound Care dialogue box, complete all necessary fields. You should defer to your agency's protocol. The following fields are required:
- Wound Name
- Wound Type (Surgical - Open, Surgical - Close, Pilonidal Sinus, Trauma, Burn, Skin Tear, Malignant, Inflammatory Vasculitis, Lymphedema, Undiagnosed, Venous Leg Ulcer, Diabetic Foot Ulcer, Pressure Ulcer (Choose Stage), Pressure Ulcer (Unstageable), Arterial Ulcer, Mixed Ulcer, Calciphylaxis, Tumour, Nectrotic/Gangrenous, Fistula, Abscess, Pyoderma Gangrenous, Neuropathic, and Other)
- Wound Status (New or Recurrent)
- Wound Acuity (Acute, Chronic, Maintenance, Slow Healing, and Unknown).
- Wound Location (Abdomen RT, Abdomen LT, Abdomen RT and LT, Abdomen Mid Line, Arm/Shoulder RT, Arm/Shoulder LT, Back RT, Back LT, Back RT and LT, Back Mid Line, Buttocks RT, Buttocks LT, Buttocks RT and LT, Chest RT, Chest LT, Chest Mid Line, Hand RT, Hand LT, Head/Neck RT, Head/Neck LT, Head/Neck RT and LT, Head/Neck Mid Line, Foot RT, Foot LT, Heel RT, Heel LT, Hip/Trochanter RT, Hip/Trochanter LT, Knee to Ankle Area RT, Knee to Ankle Area LT, Hip to Knee Area RT, Hip to Knee Area LT, Hip to Ankle Area RT, Hip to Ankle Area LT, Ischium RT, Ischium LT, Perineum, Sacrum/Coccyx, Toes RT, Toes LT, or Other)
- Frequency
- Treatment Protocol
- Total Visits Permitted
- Treatment Status
- Location Drawing
Complete any other necessary fields before clicking Ok to save the new Wound. Note that the Wound Care Entered By and Wound Care Entered fields will be automatically populated with your Employee profile name and the current date.
Should you wish to include a picture of the wound in addition to a simple drawing, you can do so using a custom report or form. In Reports (original) or Forms 2.0, whichever feature your agency currently uses, create a report called for example Wound care. Configure a picture field. You can then add this report/form to your client's profile and add a picture of their wound to it.
The new Wound profile will appear in the Client's list of Wounds under Wound Care. To review or edit the Wound details, click the View button or the Wound Name.
If you need to edit information about the Wound, click the Edit button.
Note: only the Stage, Frequency, Treatment Protocol, Total Visits Permitted, and Treatment Status fields can be edited after the Wound profile is created.
To add a Treatment, click +Add Wound Treatment.
