When creating an Automated Provider Report ("APR") for a client service, one of the two scenarios below will occur:
- The client has an active care plan with goals added. When properly configured for the client, the goals on the care plan will automatically populate in the report when creating a new APR.
- If no care plan is set up for the client, you can still proceed with creating the APR. A warning message saying "No Active Care Plan" will show up to remind users, but users can still complete, save, and send the APR without a care plan and goals.
You can use APRs with either Care Plan 1.0 or Care Plan 2.0.
Configuring goals for APRs
You can mark which goals should appear in APRs by using a care plan tag with the label APR. Your organization should add a tag with APR as the Tag Label and Care Plan Item as the Tied Entity.
Once this tag has been created, you can select the tag as APR on care plan goals and select the Service that you wish to populate in APRs. If a goal has the APR tag, it will appear in the client's APRs regardless of the goal type selected in the Goal Type Used to Populate Goals Into APRs system setting.
If you start using the APR tag along with the service to filter goals the usage of the setting Goal Type Used to Populate Goals Into APRs is completely optional.
If your agency is using Care Plan 2.0, any goals of the type selected in the Goal Type Used to Populate Goals Into APRs system setting will also appear on APRs. You can select any type in this field that you have configured as a Care Plan Goal Type.
Care plan goals of this type will flow through to an APR as long as they are either linked to the specific service you are creating the report for or if no service is selected on the goal.
Creating and sending APRs with care plan goals
When you add an APR for a client's service, active and completed goals from the client's care plan will appear in the Goals section of the Add Provider Report dialogue.
Note that for Care Plan 2.0, any goals linked to a different service than the one you are creating a report for will not appear.
The goals will be numbered in the order in which they appear on the client's care plan. Note that the order of the goals must remain the same for each APR submitted for a service and client.
A goal will be marked as New if it is the first time appearing on an APR for the client and service.
The Status of the goal on the APR will be In Progress as long as the goal has not been marked as complete on the care plan. Once the goal has been completed on the care plan, the Status on the APR will be Complete.
If you are using Care Plan 2.0, note that:
- Care plan goals will flow through to an APR if they are of the type selected as the Goal Type Used to Populate Goals Into APRs in System Settings or if they have the APR tag.
- Goals will only populate in APRs if they are not linked to a service or linked to the service you are creating the APR for.
- Goals in active or completed status will appear in APRs.
To add an outcome code for a goal, click the button in Progress Update field. If you are submitting to Central LHIN, you must select an Outcome Code from a predefined list.
Select the correct Outcome Code from the dropdown (01 Goal Exceeded, 02 Goal Achieved, 03 Less Than 50% Achieved, 04 Greater Than 50% Achieved, 05 Goal Not Achieved, or 06 Goal Not Initiated). Note that if you select 05 Goal Not Achieved or 06 Goal Not Initiated, you will be prompted to select an Unmet Reason Code (A Deceased, B Admit to Hospital, C Transferred, D Non Adherence, E No Longer Eligible, or F Other).
If you select F Other, a text field will appear to enter the reason.
When you are finished, click to save your changes to the goal.
If you are submitting to Central East or North East LHIN, you must enter your progress update in the resulting text field. You can use up to 250 characters for Central East LHIN and up to 500 characters for North East LHIN.
If you are submitting to Central West LHIN, for all reports except Personal Support enter your Progress Update in the text field provided (250 character limit) and select an Outcome (Goal Exceeded, Goal Achieved, Less than 50% Achieved, Greater than 50% Achieved, Goal Not Achieved, or Goal Not Initiated).
If the report name is Personal Support, you will only have to select an Outcome.
If you select Goal Not Achieved or Goal Not Initiated, you will also need to select an Unmet Reason (Deceased, Admit to Hospital, Transferred, Non Adherence, No Longer Eligible, or Other).
Selecting Other as the Unmet Reason will open a text field for you to enter a description of the reason (50 character limit).
Click to save your update.
If you wish to edit the Progress Update, click the icon.
To remove a goal from the report, select the Delete button.
