How do I view services or request a new service from the family portal?

The active services list in the family portal displays the services that the client is currently receiving care for. You can request a new service be added to this list directly from the family portal. Your agency will do its best to fulfill your request according to your specifications.

Choose which type of device you are using the family portal on:

View active services and request a new service (computer/tablet)

The Active Services tile can be accessed from the Home tab or the Care tab.

Click the name of a service to view more details, including the caregiver’s name, start date, end date (if applicable), service address, required skills (if applicable) and financial information such as the service code and rate.

To request service changes, click the Request New Service button the Active Services tile.

In the Request New Service dialogue box, select the Start Date on which you wish the new service to begin. Select an End Date if you have one in mind. In the Note box, describe the new service that you wish to schedule with the agency.

When you have finished, click Submit Request. The agency will be notified about your request.
