Clients and their families can complete forms created by the agency from the family portal. Active forms of type Family Portal will be accessible to portal users.
You can restrict the ability to complete forms in the family portal to certain user roles by using the Add client forms ACL in the Workflow folder for Client Portal Roles in Settings>Roles and Permissions. The Edit your own forms ACL is required to edit forms in the portal after submitting them.
To configure a form so that it is available to complete in the family portal, go to Settings>Form Settings and select +Add Form.
Select Family Portal as the form Type.
Use the Restrict Submit to field to select which client portal roles (patient, family, etc.) should be able to submit this form. If you leave this field blank, agency employees will also be able to submit this form.
Complete the rest of the required fields and click Save.
Design your form in the form builder. Once you have previewed the form and are ready to publish, select Publish.

When a family portal user submits a form for a client, the form will appear in the client's list of forms under Care Documentation>Client Forms on the client profile.
The icon indicates that the form was submitted from the family portal.
If the form contains any advanced components, portal users with permission to submit the form will be able to make updates to the client's demographics, risks, medical history, medications, vitals, and progress notes (once the form is approved). A tooltip will appear next to the portal user's name indicating that they are a family portal user for updates to medications, vitals, and progress notes submitted via a family portal form.
Depending on their role and permissions, the portal user may be able to edit the form from the portal until your agency approves the form. Once the form is approved, the user can no longer edit the form.
