Add Care Plan Update date to Care Plan Summary report

Currently only the Created Date is included on the report, which doesn't reflect that the care plan was more recently updated. If minor changes are made to care plans, staff often edit existing care plan rather than creating new version as they do for quarterly reviews or significant changes. Previously logged under CC-7242.
