Redo Equipment/Supplies Management, Create an Open Field in Medical History

Create a "DME and Supplies" field in the medical history tab that can link to the 485.  Having to add a service, then items, then an invoice when we don't bill supplies is so many extra steps.  We create an item for each client where we list all of their supplies, that makes it much easier to manage changes and cut down on the amount of time spent building the invoice out.

*In the meantime, make the description box expandable so we can see what we are typing.
